In Ennies Lobby Arc, Luffy first uses Gear 2: Rubber blood vessels against a CP9 member, Blueno. This happens in manga chapters 387 and 388 of episode 272 of volume 40.

Second Gear increases his speed and strength as he increases his body’s nutrients, blood vessel size, and amount of oxygen. After the time skip, Luffy can no longer rely on his legs to activate Gear 2, but can activate them anywhere in his body now. Initially, this technique activated Luffy’s strength, speed, and agility.
